The new-patient form
01After you book your first visit
The first form collects what the clinic needs to know before it sees you for the first time. It arrives by email once the appointment is booked, and it can be filled in from a phone, a tablet or a computer.

02Once your appointment is scheduled
The screening questions are answered online. Doing them before the appointment means you can read each one properly instead of working through a list at the desk.
The day-of check-in
03The morning of your visit
The last one is the check-in itself, and it comes by email on the morning of the appointment. Doing it early means less of the visit is spent repeating information at the desk.

The new-patient form, online
The form asks what the clinic needs to know before it sees you for the first time. Filled in on a phone, a tablet or a computer, it checks each answer as you go and reaches the office the moment you send it.
The printed one asks the same questions in the same order, so a form finished at the kitchen table is a form the desk can file as it is. Bring it with you.

Tell us what has changed
Six questions about anything new in your health since your last visit — an illness, a medication, a heart problem, a pregnancy. Two minutes online, and your dentist reads the answers before you sit down.
Sent online, it reaches your chart straight away. Bring the printed one to the desk if you would rather write it out by hand.
Coming in without an appointment? Walk-ins are welcome when the chair is free, and the desk goes through the forms with you when you arrive.
If you booked and nothing has arrived, check the address you booked with first. If a question on a form is unclear, the desk can sort it out.
