Once your appointment is scheduled, you will be asked to fill out this form to assess the type of PPE that will be prepared in advance for your appointment.
COVID IN-OFFICE FORM
On the day of your appointment, you will be asked to fill out this form prior to entering the office. This form acts as an update to the information you would have already provided in the “COVID Form” submitted beforehand.
