Recall exams, bitewing x-rays and cleanings (1 unit polishing + 1 unit scaling) — once per benefit year from September 1

Fluoride treatments twice per benefit year (19 and under)

Complete oral exams, full-mouth and panoramic x-rays — once every 3 years
